Please fill out for each auction item. Please note: a receipt will be sent to the donor - i.e. Chateau Ste Michelle if they donated wine to you. Please provide as much detail as possible - i.e. "5 bottles of 2018 Chateau Ste Michelle Cabernet Savignon" (vs. just "wine basket"). Also, make sure to include any restrictions (specifically for trips/stays if there are black out dates) and you must include an Estimated Value. Thanks!
